If your answers to the questionnaire indicate that you may be eligible to participate in the Study, you will have the option to enter your name and contact information so that a Study centre can contact you to discuss your possible participation in the Study.

How we will use your personal data
This site does not use cookies or track your IP address or device information.
If you complete the questionnaire but do not submit your name and contact information, this site will not store, process or transfer any of your personal data.
If you submit your name and contact information, they will be transmitted together with your questionnaire answers (including information about your health) through a secure portal to your local Study centre, which is the medical institution identified in the questionnaire. An authorized representative of the Study centre will then contact you to discuss your possible participation in the Study.
Access to your personal data
Only the Study centre will have access to your personal data. The information will not be accessible to us or to any third party, except for IT personnel of our service provider who are contractually required to keep such data confidential and may access it only for technical support purposes.
The information will be retained in our secure portal until the recruitment phase of the Study has been completed. It will be deleted from the portal within six months after completion of the recruitment phase. The Study centre will be permitted to keep a copy of the information in their study files for the duration of the Study and so long thereafter as they keep Study files, in accordance with Study centre policies and applicable regulations.
How to access or delete your personal data
If you submit your contact information, you will receive an e-mail that includes contact information for the Study centre. To access or delete your personal data you may contact the Study centre. You may also contact the Study centre to withdraw your consent to the further processing and use of your personal data submitted through this website.
